A hospitalist is a physician who specializes and works only in a hospital and nursing home. Your hospitalist team is available around the clock to provide patients with exceptional care.
With compassion the OptiMed team serves patients of several local primary care physicians. Additionally, we care for those patients who have a doctor outside the area, as well as those patients who do not have a primary physician. We also care for all patients in the nursing homes we serve.
Optimed is a team of highly qualified, compassionate medical providers. All our physicians are board certified specialists, primarily in Internal Medicine. Our team also includes highly experienced and qualified nurse practitioners specializing in Internal Medicine and Family Medicine. Our physicians see patients in the hospital and nursing homes we serve.
Most of our admissions and consults arrive through the emergency department. If your primary physician, specialist, or emergency department physician believes that you need to be hospitalized, he or she will coordinate your admission with our hospitalist team. If you are admitted through the emergency department, your physician will be notified. Once admitted, the hospitalist team will supervise your care, see you daily, and give you quality care. The hospitalist team will:
• Assess and treat your medical condition
• Coordinate your hospital care
• Review your progress with your physician
• Establish a relationship with your family
• Transition your medical care back to your primary physician, who will resume responsibility for ongoing outpatient treatment, after you are discharged
Our medical providers will send copies of your records to your physicians, and be in contact with them as needed, relaying the details of your care and discussing your management.
Your hospitalist may consult specialists, such as cardiologists, pulmonologists, gastroenterologists, and others as needed, to assist in your care.
A hospitalist will see you 7 days a week and one is on call 24 hours a day.
Unless against your wishes, your family will also be engaged in your care, and your hospitalist is available throughout the day and night to answer questions that you or they may have.
Your hospitalist will discuss your discharge plans, medications, and instructions with you. Your primary physician will receive a summary of your stay, including diagnoses, studies, procedures, and medications that have been ordered or provided, as well as recommended follow-up visits and treatments.
Yes. Optimed is an independent medical group. As such, you will receive a bill from our group separately from the hospital. We will submit all claims to your insurance company. First and foremost, high quality care is available to you 24 hours a day, seven days a week. Hospitalists specialize in the hospital management of acutely ill patients, and are especially suited to their care. They will see you as often as needed. Additionally, because hospitalists are based at the hospital, they can discuss your care with sub-specialists and ancillary staff, and review results of your tests in a timely fashion. Hospitalists are easily accessible in emergencies and decisions about you can be made quickly as situations arise. This will result in more rapid and accurate clinical decisions, prevent complications, reduce your length of stay and cost, and produce better outcomes with fewer re-admissions.
